Research how to videos. Decided problem was probably the phase control board, but in the interest of time and since the additional parts did not cost much, I also replaced the control plate and motor brushes. Note, the control plate is attached by three screws, two of which are spring loaded. Hence, their adjustment is critical. I took close in photos of their extension or placement using a ruler so I could ensure correct reinstallation of the the replacement plate.

Phase control board was easy, The control plate took some time to adjust to get all speeds to work. It seems that you have to adjust top left screw to adjust slow speed first and than the right screw for speed. The bottom screw has to be just right for shut off and on. I noticed if you just put control in it doesn't work, it has to be adjusted

After a power surge, my mixer only operated at high speed, no matter what speed I had it set.

Take off the back of the mixer, then unscrew the screw holding the phase control board in place. Disconnect four wires. Connect the wires to the new phase control board, then replace the screw and the back of the mixer. Mixer is working great now!
